Almost nobody books their first appointment with us because they want therapy. They book because they have not slept properly in months. Then, somewhere in the first few sessions, the thing behind the sleep comes into view: a worry that never switches off, a mood that has been flat since spring, a loss that is still sitting on the chest, a memory the nights keep returning to. Sleep is the door most people come through. This page is about what is on the other side of it — and about the counseling half of our name.

What We Treat

Each of these has its own page describing what treatment looks like here. Every one of them is delivered by clinicians who are also trained in Cognitive Behavioral Therapy for Insomnia, so if sleep is part of your picture it is treated in the same plan rather than referred elsewhere. The numbers are how many of our North Carolina clinicians currently taking new patients list that problem as a focus.

Anxiety runs through nearly everything above — 5 of our 5practising clinicians list it — and is treated within whichever course of care fits you. 1 clinician also lists substance use, which we work with when it is tangled up with sleep or mood.

How Online Therapy Works Here

An evaluation, not an intake form

The first appointment is a conversation with a licensed clinician about your nights, your days and what you want to be different. You leave it with a working understanding of what is going on and a plan, not a questionnaire.

Weekly, by video

Sessions run weekly to begin with, usually 50 minutes, over a secure video link from wherever in North Carolina you are. Therapy for the conditions above is measured in months; your clinician will give you a realistic estimate rather than an open-ended one.

Licensed for the whole state

We see most patients via telehealth with North Carolina-licensed clinicians, and a limited number of clinicians also see patients in person. A Murphy address and a Manteo address get the same care, from someone licensed by the same board.

One plan, not two referrals

If sleep is part of the problem, CBT-I goes into the same treatment plan. You will not be sent to a separate sleep clinic for the nights and a separate therapist for the rest.

Insurance and Cost

Plans we bill

Therapy here is ordinary outpatient behavioral health care, covered by most North Carolina plans the way a specialist visit is. We are currently in network withAetna, Blue Cross Blue Shield, Cigna / Evernorth, Medicare, UnitedHealthcare / Optum, UnitedHealthcare Medicare Advantage and Veterans Affairs Community Care Network.

What you will pay

Your share is set by your plan — a copay or your deductible — and we check it before the first session rather than after. Veterans using community care and Medicare Advantage members need the referral or authorization in place first; tell us when you book. Not on a plan we take, or unsure? Ask and we will find out for you.

Common Questions

Do you take Medicare for therapy?

Yes. Medicare is one of the plans we bill in North Carolina, alongside Medicare Advantage through UnitedHealthcare and the commercial plans listed on this page. Medicare covers outpatient therapy with a licensed clinical social worker or counselor the same way it covers other outpatient care, with the usual Part B cost-sharing.

What if the first clinician is not the right fit?

Say so, and we will move you. Fit matters more than almost anything else in therapy and nobody here takes it personally. Because our clinicians are licensed across North Carolina and see most people by video, switching does not mean starting over with a different practice or driving somewhere new; it means a different face on the same screen next week.

Is this therapy, or coaching?

Therapy. Every clinician on our North Carolina roster holds a state clinical license — LCSW or LCMHC — which means they are qualified to assess and treat diagnosable conditions, bound by clinical ethics and confidentiality rules, and billable to insurance. Coaching is none of those things. We do not offer coaching, and we do not describe what we do as wellness.

How do I know which page — or which problem — is mine?

You do not have to decide before you book. Most people arrive with a symptom, not a diagnosis, and untangling which problem is driving which is what the first appointment is for. The pages linked above describe what each kind of treatment looks like; if one of them reads like your life, start there, and if none does, start with the sleep and let the evaluation sort out the rest.

Do you see teenagers?

Our North Carolina practice is set up for adults. If you are looking for care for someone under eighteen, tell us when you get in touch and we will say plainly whether a clinician on our roster is the right fit, rather than leave you to find out at the first session.
